Copy it exactly as captioned, including LLC or 'as assignee of'. Often a debt buyer you have never heard of.

This is what decides whether the debt is too old to sue over. Never make a new payment on an old debt, it restarts the clock.

Your defenses
Pick everything that is true. Each one becomes a numbered affirmative defense in your answer. A defense you leave out now is usually waived, so err on the side of checking it.

What happened, in your words
A few sentences the judge can read: who the original creditor was, what you dispute, what you paid, what they never sent you. Plain English, no legal words needed.
